New Patient Forms

Chiropractic Associates of Michigan offers our new patient forms online so they can be completed it in the convenience of your own home or office.

• If you do not already have AdobeReader® installed on your computer, Click Here to download.
• Download the necessary form(s), print it out and fill in the required information.
• Fax us your printed and completed form(s) or bring it with you to your appointment.

Please contact your Chiropractic Associates of Michigan Front Desk Caregiver to determine which forms are suitable for your condition. Download the required forms and fill in as completely as possible. You can fax them directly to the clinic at 586-293-0840 in advance of your appointment, or bring them in with you on the day of your appointment. If you have any questions or difficulty when you are filling out the forms, simply call the clinic for clarification, or bring with you for your first visit and our staff will be happy to help you fill them out.
